- Hand-print your name, address (NO P.O. BOXES, except in ND), city, state, zip code, daytime telephone, e-mail address (optional) on the official offer form (available at participating Mobil and Exxon gas stations);
- To receive a $5.00 rebate check, send original register receipt dated between June 1, 2008 – July 31, 2008 showing proof of purchase of an oil change using Mobil 1 product with completed offer form to:

Offer Conditions: Residents of North Dakota may use a P.O. Box for their address. All rebate requests must be received by August 15, 2008. Limit 1 rebate per person/household. Allow approximately 6-8 weeks for delivery following receipt of all required documentation. Void in Rhode Island, Puerto Rico, the U.S. Virgin Islands and other U.S. territories and where prohibited, restricted or taxed. Offer valid only to individual consumers; requests from groups, clubs or organizations will not be honored. Offer cannot be combined with any other offer, coupon or discount card.
